Dave is a 55 year old male who is presenting with depression. He was furloughed during Lockdown and has yet to go back to work. He says that he just doesn’t feel like there is any point to anything anymore. He feels hopeless and anxious all the time and constantly feels sick and gets headaches. He is divorced and his children live four hours drive away with their mother. He has seen them twice since lockdown and does speak to them via Facetime, but feels that they are not very interested in speaking with him – they seem anxious to get back to whatever they were doing before he rang. He used to have a busy life before Lockdown – work, gym and pub nights with his mates. All that went instantly in March and now he struggles to find anything meaningful to do to fill his days. His sleep is poor and erratic – he stays up watching ‘rubbish’ on TV and sleeps late in the day. His diet is poor – he can’t be bothered to cook or shop.

Write a report outlining how you would work with Dave with the following elements:

         Create a suggested Formulation to explain his depression and explain the elements in this formulation (Formulation plus 750 words to explain it). This could be the 5 Ps, five areas or a Longitudinal formulation.

         Discuss what basic tools you might use to treat his depression and why (eg thought-catching, exposure therapy, behavioural activation etc).  (max 1000 words). You should use the literature where appropriate to support your arguments in this section.

Is the amount of detail given of the theory and relevant literature appropriate in terms of the aims of the coursework? Can the reader see a logical connection between the theory described, its application to the situation in the question and the conclusions drawn? Is the Formulation clearly explained?

Style
Is the writing elegant and economical with very few grammatical and spelling errors? Is technical language used appropriately? Does the Formulation look clear?
