Care coordination is becoming more and more prevalent in the work that nurses do, and your presentation supports your understanding of the importance of care coordination to the role of the nurse. Your presentation delivered key care coordination information to your colleagues, and I encourage you to think about how you can use this presentation in your own practice. Would your facility benefit from this information? Could you share this with other nurses or administration? I think you did a very nice job presenting the basics of care coordination. I like that you provided key evidence from the literature, like your AHRQ reference, to provide key insight to your peers.
