OPERATIONS MANAGER
Overview- high quality home care to seniors and people living with disabilities in the community and beyond. Provides personal care, domestic help, community engagement, dementia care, palliative care, support coordination, 24/7 care, respite and much more.
My Role- Provided day to day operational and administrative management of all client services. Proactively build and maintain positive collaborative relationships with Participants, families, stakeholders, supports coordinators, plan managers, LAC’s and NDIS planners. Responsible for rostering, reviewing and approving staff timesheets and invoicing clients in Home Care Package and NDIS using a care management platform such as SHIFTCARE and FLOWLOGIC. Responsible for the implementation of systems and processes that place the client at the centre of service delivery, exercising maximum choice and control.
SUPPORT COORDINATOR
Overview– Support Coordination is an NDIS funded Support, designed to assist NDIS participants to make sure they get the most out of NDIS funding package.
My Role- Supporting participants and their families to understand their NDIS plan. Experienced with complex needs participant cases. Providing regular communication/reports with the participants, their families, and National Disability Insurance Agency (NDIA). Liaise with community organisations to see if their services are a good fit for the participants. Prepare NDIS plan reviews to communicate participants goals and aspiration. Home and community visits and case management. Refer to the NDIS price guideline to check service agreements.
I have offered strong relationship and community capacity building. Improved engagement and quality of life for people with a disability in addition to supporting support workers through accountable and engaging leadership which has boosted morale, motivation and ultimately, outcomes and positive experience for participants.

Key Selection Criteria
—–I have extensive knowledge of using care management systems such as SHIFTCARE and Flow logic. It’s the same as the TRACCS Client system. I am also experienced in using NDIS Provider Portal, PRODA, Department of Communities OASIS, Disability Services OD
Bachelor in Human Services double major in Children and Family Studies and Human Resource Management, Bachelor of Nursing and Diploma of Leadership and Management /Diploma of Human Resources
