In this module, you will work as a team and prepare a report on the incident caused by the Therac-25 at the East Texas Cancer Center in March of 1986.

As a team,

  1. Review the Reading & Multimedia Materials provided as part of this collaborative project,
  2. Discuss the role of the technician, the designers and engineers of the Therac-25, the manufacturing company, and the FDA in this case,
  3. Identify four major factors that contributed to the incident,
  4. Classify each factor according to the Socio-Technical System,
  5. Discuss whether similar factors contributed to the incident which took place at Three Mile Island, and
  6. Prepare your report presenting your analysis of the case based on your team discussion.
