Health History and Family Genogram Clinical Project-Scholarly Paper

Scholarly Paper – After completing the health history interview and genogram, describe the following in a 3-4 page (excluding cover page and reference page), APA formatted paper:

• Section One: Introduction to the paper’s content
  • General statements on the benefits of evaluating family genetic disease patterns.
  • Information/summary of the sections of the paper that will be addressed.
• Section Two: Evaluation of Family Genetic History
  • Describe patterns of disease that extend through the generations on both sides of the family.
  • Choose the two most prevalent diseases or illnesses that extend through the generations on both sides of the family.
  • Analyze potential familial effects related to the prevalent family diseases on the participant’s health using support from scholarly literature.
• Section Three: Evaluation of Participant’s Health Behaviors
  • Evaluate the participant’s current personal health behaviors.
  • Indicate the current positive and/or negative personal health behaviors demonstrated by your participant.
• Section Four: Planning for Future Wellness
  • Describe what you will teach the participant about what he/she can do to prevent these diseases/illnesses. Include scholarly support.
  • Indicate what modifications are necessary for maintenance of their personal wellness and disease prevention (of the identified patterns of illness and current health behaviors). Include scholarly support.
  • Discuss your finding with the participant and discuss their reactions to your findings and suggestions for health promotion and disease prevention.
• Section Five: Conclusion
  • Summarize findings and recommendations.

Format:

1. Length of paper: 3-4 pages not including title page or reference page.
2. A minimum of three scholarly references other than your textbook: References must be no older than 5 years, unless a valid rationale is provided.
3. Title page, body of paper, and reference page must be in APA format.
4. Must correctly cite ideas and information from readings or other sources.
5. Grammar, spelling, punctuation, and citations are in accordance with APA format.
6. Include headings of the five sections of the paper (Introduction, Evaluation of Family Genetic History, Evaluation of Patient’s Health Behaviors, Planning for Future Wellness and Conclusion), within the body of the paper.
