Through this interview, you would like to gather information about the EMR system that the provider is using.  You should specifically gather information about 

1. The functionalities that the EMR serves to the hospital and how have these functionalities changed or evolved over the duration that they have had this EMR. 
2. The advantages of using the EMR system
3. The challenges of using the EMR system 
4. The perception of the EMR among the clinicians, healthcare workers and patients. 

Prepare an interview guide with 5 questions. 

Document the findings of the interview through a narrative of up to 5 double-spaced pages. I am providing an example of an interview (Physician Interview.pdf  Download Physician Interview.pdf on a different topic )  of what I expect in the narrative.
