Note: Before you begin this stage, review the HINF “Pharmacy Mini Clinic” Course Project Rubric.

This week, you will continue your Course Project by completing Stage 4. This stage will be a component in your “Pharmacy Mini Clinic” Course Project and will be due on Day 5.

To complete this stage of the Course Project, you will need to do the following:

Open the database created in Week 3, then use the information from the Week 2 PowerPoint slide to create a fourth table in your Week 3 database.
Add all attributes as shown in the Week 2 template. Make sure that the data types for the attributes that will be holding foreign keys match the data types for the corresponding primary key attributes in the three previously created tables. Remember, in the table that you are creating, you are using a surrogate primary key that will require the auto value functionality. Designate the primary key for the table.
Create the relationships between the tables using the Relationships tool in a Microsoft Office program .
Finally, populate the table that you created with sample data. (Note: You will need to open the other three tables to get the pertinent primary key data for the foreign keys you are entering in your new table.

Be sure to include:

At least one vaccine issued for each patient
At least three patients who received more than one vaccine
