Based on module 3 readings and materials, additional self-selected readings and student experiences, please reflect and thoughtfully engage in the discussion.
Initial Post = 70 points
Initial post due by November 20 by 11:59 PM. Be clear and concise, but no word limit.
1. What (description) What is the adverse event reporting process in your organization? Is there a written policy? How do new nurses in your clinical setting learn about what constitutes an adverse event? How do they learn about the reporting process?
2. So What (reflection using theory and knowledge) – Based on module 3 incident-reporting readings, additional self-selected readings and student experiences, what do think about the adverse event reporting process at your organization? In your clinical setting, is the process adequate, are all clinicians engaged, and is there sufficient corrective action taken? Is there a feedback loop to the reporter? Where are the strengths, what are the gaps? Cite at least 2 peer-review articles to support your position.
3. Now what (action) – As a DNP prepared leader, identify two strategies you can use to take an active role in improving the adverse event reporting process. Cite at least 1 peer-review articles to support your intended actions.

Grading Criteria: The DB is considered part of the scholarly contributions of the student to the course and should: 1) demonstrate integration and synthesis of patient safety and health care quality knowledge and principles from the text (if used include page numbers), learning module readings or other appropriate sources; 2) offer a different and unique, but relevant, insight to the discussion; 3) move the discussion and analysis forward to generate new insights and ideas; 4) ask probing questions; 5) use logic, evidence, and creative thinking, and; 6) is more than an expression of an opinion or feeling. Points will not be earned for expression of feelings or simple expressions of agreement or disagreement.

It must be evident that the student is engaged in the discussion and not merely clustering responses in one day. The responses must be in paragraph form, reference appropriate literature to support your position and be grammatically correct.
